LONG-TERM BOARDING CONTRACT

 

 

This is a contract between Center for Avian Rehabilitation & Education Inc. (“CARE”) and the pet owner (“Owner”) whose signature appears below.

 

  1. Owner agrees to prepay monthly the rate for boarding in effect on the date pet is checked in to CARE ($300 per month), and further agrees to pay for all costs and charges for any special services requested, and all veterinary costs for the bird during the time the bird is in the care of CARE, including grooming.

 

  1. Owner understands that their pet may be picked up anytime during CARE’s open hours.  Boarding charges start on the day the pet is brought to CARE, regardless of time of day.

 

  1. CARE will exercise responsible and reasonable care for Owner’s boarding pet.  Owner agrees to be solely responsible for all acts and behavior of pet while it is in the care of CARE. 

 

  1. Owner verifies that their pet has had a wellness exam performed on their pet within the last year.

 

  1. Owner understands that if their bird has full flight, that bird will not be allowed out of its cage for the duration of boarding. 

 

  1. If your pet becomes ill or if the state of health requires professional attention, CARE, in its sole discretion, may engage the services of a veterinarian or administer medicine, or a special diet, or give other requisite attention to the pet, and the expenses will be paid by Owner.  In the case of a medical emergency, the maximum dollar amount that Owner authorizes to be spent on veterinary care is $____________.  If the pet dies while boarding at CARE, CARE will immediately attempt to notify Owner via the emergency contact information left by Owner.  The body will be stored at either CARE’s veterinarian, or at CARE, per Owner’s instructions.

 

  1. Owner agrees that the pet shall not leave CARE until all charges are paid to CARE by owner.  If the pet has not been picked up within ten (10) days of the agreed upon pick-up date, and owner has not contacted CARE regarding a new pick-up date, the pet becomes the property of CARE. 

 

  1. All checks returned for insufficient funds or otherwise will be subject to a $25.00 charge.

 

  1. CARE is not responsible for any damage or destruction of any property left with the pet.
